LA GRANGE – Home and business owners whose property sustained damage in last week's storms are asked to fill out an Initial Damage Assessment form.
The Cook County Department of Homeland Security and Emergency Management is collecting information from homeowners, renters and businesses related to flood damage and costs from local jurisdictions in order to provide it to the Illinois Emergency Management Agency. The information will be used to preliminarily determine whether the area is eligible for federal disaster assistance. Updates regarding this process will be provided to residents as they are received.
The form should be filled out and emailed to La Grange Fire Chief Bill Bryzgalski at email@example.com, or faxed to 708-579-9747. It can also be dropped off at the La Grange Fire Department, 300 W. Burlington Ave.
Completed forms are due by noon on Friday, April 26, in order to meet the county's deadline. For questions regarding the form, please contact Chief Bryzgalski at 708-579-2338.
